Ann Theodore Foundation Scholarship- New York

The Ann Theodore Foundation Scholarship- New York opens the door to post-secondary education opportunities for youth and adults. This program is designed to help residents from North Babylon and Roosevelt, New York attend and graduate from post-secondary institutions including two-year schools, trade schools, and four-year colleges. The program serves low-income high school seniors and adults who have a desire and a plan to achieve a college degree and for whom financial support along with mentoring and guidance will enable them to move ahead with their dreams of attaining a college degree. If accepted into the program, students receive scholarships of up to $25,000 annually, for up to four years, plus individual mentoring and funding for enrichment opportunities.

Required Documents

  1. Complete Online Application Form
  2. Financial aid award letter (if available)
  3. High school transcript
  4. College transcript for students who are already in college
  5. Letters of Recommendation – the email addresses of TWO people the applicant has known for at least one year through school, work, or a volunteer experience.
  6. Essays – Essay questions are located within the online application form and may be found on our website. There is no perfect essay response. The essays are an opportunity for us to understand your goals and experiences.
  7. FAFSA Student Aid Report (SAR) – This document will be approximately five pages long.  You must submit the ENTIRE SAR document. Please note: You will be asked to enter your “Adjusted Gross income (AGI)” into your application.  If you do not have access to this figure in your FAFSA SAR, you can find it on line 37 of your 1040 tax form.  If you or your family uses another version of the 1040 form, you can figure out how to find your AGI here:
